The following form should be completed by a Williston Northampton School faculty member or host. The school’s Registrar will contact you after your form is submitted and reviewed.

Please provide a W-9 form to the visiting speaker, if an honorarium is requested. The form needs to be mailed directly to the business office by the visiting speaker.
